Report a bug moves into the user menu

Today the Sentry "Report a Bug" pill floats bottom-right over every page and covers whatever control sits there. The proposal removes the pill and adds a Report a bug item to the avatar menu, straight after Help & Support. The feedback form itself does not change. Each state is shown at phone width (375 px) and at desktop width (1280 px, scaled to fit). Every frame is live: scroll it, type in it, open the menu.
