CAS-2198: the Sentry "Report a Bug" pill floats bottom-right over every page and covers controls there. The entry point moves into the user avatar menu.
This file shows the proposed change. No pill. Open the avatar menu (top right): Report a bug sits after Help & Support and opens the same Sentry form. Esc or Cancel closes it; Send shows the thank-you toast.
The form is a mock of Sentry's own form; on build it is Sentry's form, unchanged. See PROTOTYPE.md.
